Application for S.1 Discretionary Places (2012-2013)

中一自行分配學額申請 (2012-2013)

1. Application for S.1 Discretionary Places (2012-2013) will be accepted between 3 January 2012 to 20 January 2012. The number of S.1 Discretionary Places(2012-2013) is 54.

2. The application form can be obtained from the school in person or from the school website (www.spslt.edu.hk). School Office hours are: 8:30 am.- 5:00 p.m. (Monday-Friday) 8:30 am – 12:00 noon (Saturday)

3. Applicants have to submit completed application forms from EDB and the school together with photocopies of the following documents:

l Birth certificate and identity card

l Certificate of Baptism (if any)

l School Reports of Primary 5 and Primary 6 ONLY,

l Certificate / supporting documents for outstanding activities

l Two stamped and self-addressed envelopes

All documents should be arranged in order, stapled and enclosed in an A4 size envelope before submitting to the school on or before 20 January 2012.

Applicants should receive from the school the Parent’s Copy bearing the school chop, school name and school code for retention and record.

*Recommendation letters from principal/teachers from primary schools are not required.

4. Applicants within the top 250 in the Discretionary Place Rank Order (from EDB) will be invited for an interview. A letter informing details of the interview will be sent out in Mid March 2012. Applicants not selected for interview may also be considered for admission. Parents do not need to call the school for enquiry or information.

1. 本校將二零一二年一月三日至一月二十日接受二零一二至二零一三年度自行分配學額申請。本年度自行分配學位數目為54。

2. 申請表格可於本校網頁(www.spslt.edu.hk)下載或親臨本校索取。校務處開放時間為﹕

8:30 a.m. – 5:00 p.m. (星期一至五) 8:30 a.m. – 12:00 noon (星期六)

3. 請於二零一二年一月二十日或以前，把填妥之申請表格(學校及教育局表格)及下列文件之影印本，依次序填妥後放在A4大小的公文袋內交回本校。
有關文件之影印本，包括：

l 出世紙及身份証

l 領洗紙 (已領洗天主教徒)

l 小五及小六之成績表

l 最近期或表現最優異之校內或校外活動證書 / 獎狀

l 並請附貼上郵票之回郵信封兩個。

*申請者應取回已蓋上校印名稱及編號的「家長存根」以作存照。

*申請人毋須提供小學校長/老師之推薦信。

4. 本校將於二零一二年三月中寄發面試通知, 約見教育局提供成績次第表內首250名申請人。本校亦會考慮沒有面試的申請人，

家長毋須致電學校查詢。
